Participation Forms
All student-athlete clearances will be completed online.
Please see the links below for registration, directions to complete registration, and YouTube video (if desired) for more information regarding student-athlete clearances. If you do not have Internet access you may contact the school to use a school computer to register. After completing the process an email will be sent to the email address that was used to register explaining the next steps in the clearance process. Clearance will not be granted until the items below are either given directly the Athletic Director, Mr. Sheriff at a per-arranged time or uploaded during the registration process (best option).

- Signed Permission Form - will be in your email after you complete registration,
- Physical Exam - you can download a copy of the form from the clearance website and fill out both sides (parent and Dr.),
- Copy of Insurance Card - may be uploaded online but MUST have student's name on the card or state family coverage.

For those individuals that do not have private insurance. This is the insurance that the district has a contract with. The insurance is good from July 1 to June 30 of the following year.

Physicals If you need a physical we recommend visiting West Oaks Urgent Care. It is a walk-in Medical Clinic that offers quick physicals with no insurance or appointments needed! Very cheap, affordable, and a quick way to get your physical for season.
